By the Bay Health offers a comprehensive range of care services designed to support people at every stage of life and health. These services include hospice care, palliative care, home health care, pediatric care, grief support, and community-based programs. Our goal is to provide compassionate, personalized care wherever patients call home.
Palliative care focuses on improving quality of life for people living with serious illness and can be provided alongside curative treatments. Hospice care is specialized support for individuals facing a life-limiting illness when the focus shifts from curing the illness to maximizing comfort, dignity, and quality of life. Both approaches provide physical, emotional, and spiritual support for patients and families.
Some services, such as hospice and home health care, require a referral from a physician or healthcare provider. However, you do not need a referral to contact us with questions. Our team can help determine what services may be appropriate and guide you through the next steps.
Absolutely. We believe the best care happens through partnership and communication. Family members and caregivers are encouraged to participate in care planning, ask questions, and work closely with our care teams to ensure that care aligns with the patient’s goals, values, and preferences.
Caring for a loved one can be emotionally and physically demanding. By the Bay Health offers education, guidance, emotional support, respite resources, and grief counseling services to help caregivers and family members feel supported throughout the care journey and beyond.
Many By the Bay Health services are covered by Medicare, Medi-Cal, private insurance plans, and other healthcare benefits. Coverage varies depending on the service and individual circumstances. Our team can help patients and families understand their benefits and navigate the financial aspects of care.
Every person’s journey is different, and their care should be, too. We listen to each patient and their family to understand what matters most to them—from comfort and independence to meaningful routines, relationships, and personal wishes. Rather than taking a standardized approach, our care is guided by each patient’s priorities and choices, helping preserve what they value most throughout their journey.
